Allwright Bourke Prospective Client Questionnaire
  • Welcome to Allwright Bourke. To ensure that we can assist you in an efficient way - we ask that you take the time to complete this intake form.

  • At {lawFirm408}, we understand that ending a relationship can be a confusing and stressful time. Our job is to help guide and empower you with the right strategies and advice, so you can make the best decisions for your future.

    To help us, we use this secure online questionnaire provided by Navio to understand some of the important details of your situation. Completing this questionnaire will help us be better prepared for an initial consultation and avoid spending valuable time asking background questions. We can then focus on providing practical advice and strategies specifically related to your situation and your family.

    This questionnaire should take 15-30 minutes to complete and asks for details about you, your former partner, and your relationship together. Not all questions will be relevant to you - please leave any question blank if you are unsure or don't know the answer. You will be able to monitor your progress through the questionnaire with the progress bar found at the top of each page.

  • Your Data

  • Our online questionnaire is provided by Navio, which securely holds the information you provide.

    Prior to meeting with you, we need to complete our mandatory conflict checks and confirm that we are in a position to represent you. This is because we cannot represent both parties in a family law matter. We can only represent you if we are not already in contact with or representing the other party in your matter.

    When you complete this online questionnaire, Navio will provide us with the following information to enable us to complete these checks:

    - your and your former partner's names (including former names);
    - dates of birth; and
    - addresses.

    Until our checks have been completed, no lawyers or other personnel at our firm will be able to access, view, download or otherwise receive any other information that you provide in the questionnaire.

    If, following our checks, we are in a position to represent you, we will receive the remainder of your information and we will be in touch to book your first consultation with us.

    If we are not able to represent you, Navio may be able to refer you to another family lawyer from its network of leading family lawyers across Australia.

    Please note that your use of this online questionnaire is not intended to, nor will it, create any relationship of client and solicitor between you and {lawFirm408}, or you and Navio.
